A Matter of Balance (Clay and Platte Counties, MO)

Goal: The goals of the program are to help participants learn how to view falls and the fear of falling as controllable, set realistic goals for increasing activity, change their environment to reduce fall risk factors, and exercise to increase strength and balance.

Impact: After completing the program, 97% of participants are more comfortable in talking about falls, 95% have made environmental changes, and 99.5% plan to continue exercising.

ACCESS (Appropriate Clinical Care Engaged in a School Setting)

Goal: The goal of the ACCESS program is to reduce barriers to mental/behavioral health and wellbeing for children in Belton by providing quality care through free school-based therapy services (provided by licensed therapists, using evidence based practices) for our BSD students and educate our community about mental health and wellbeing to reduce stigma.

AccessHealth Spartanburg (Spartanburg County, SC)

Goal: AccessHealth Spartanburg's mission is to improve access to healthcare for the uninsured of Spartanburg County, SC.

Impact: AHS reduced inappropriate emergency room use and inpatient caseloads while increasing access to healthcare services for its clients in Spartanburg County. Furthermore, the organization reported a $17 to $1 ROI.

Addressing the Social Determinants of Health through the Alameda County, California, Place Matters Policy Initiative (Alameda County, CA)

Goal: The goal of the Place Matters initiative is to improve the health of participating communities by “addressing social conditions that lead to poor health” through “identifying the complex root causes of health disparities and defining strategies to address them.”

Impact: AC Place Matters has identified 10 best practices that could help other programs address social determinants of health: find and foster strong leadership, dedicate staff resources to the work, engage staff from across the local health department, contribute to building grassroots power, address root causes, partner with community organizations and leaders, partner with government institutions across sectors, work reactively and proactively, build capacity, and use tools that ensure a focus on health equity.

Adopt a Native Elder Program (Arizona and Utah)

Adopt-A-Ship Program (USA)

Goal: The goal of this program is to introduce teachers and students in grades 5 - 8 to the people, vessels, and activities of the American Merchant Marine, and to foster an interest in geography, history, transportation, trade, science, math, and English.

Adult Literacy Media Alliance (ALMA) (USA)

Goal: ALMA’s mission is to help adults gain basic reading, writing, and math skills they need to achieve their educational, career, and personal goals.

An Effective Practice meets the following HCI criteria:

  1. The program description includes at a minimum: the sponsoring organization, program goals, program implementation steps, and outcomes that have demonstrated program success and/or promise in achieving the program goal in one or more localities.
  2. The results from an evaluation of the program include quantitative measures of improvement in outcome of interest (i.e. increase in voter registration, not just delivery of voter registration drive) and/ or the outcome measure is increased or improved compared to the baseline measurement or the control group but the difference is not statistically significant.

A Good Idea meets the following HCI criteria:

  1. The program description includes: the sponsoring organization, program goals, program funding source, program implementation steps and outcomes.
  2. The program evaluation is limited to descriptive measure(s) of success/accomplishment (i.e., program participation rates, number of services/education sessions/radio messages provided). Note: oftentimes, the program has been newly implemented and a program evaluation has not yet been conducted. Programs that have not yet been evaluated, but which show promise in improving health or quality of life, are classified as Good Ideas until an evaluation is conducted.

An Evidence-Based Practice meets the following HCI criteria:

  1. The program description includes at a minimum: the sponsoring organization, program goals, program implementation steps, and outcomes that have demonstrated program success in achieving the program goal in one or more localities.
  2. The results from an evaluation of the program include quantitative measures showing improvement in the outcome(s) of interest after the implementation of the program (i.e. increase in smoking cessation, not just the delivery of a smoking cessation program). The outcome measure(s) is/are compared at relevant time periods before and after the intervention or program implementation. Alternatively, the evaluation study compares the outcome(s) between an intervention group and an appropriate control group.
  3. The study is of peer-review quality and presents numbers in a scientific manner; measurements of precision and reliability are included (e.g. confidence intervals, standard errors), results from statistical tests show a significant difference or change in the outcome measure(s), and relevant point estimates and p-values are presented. Note: if the results from an evaluation of a program are presented in a scientific manner and the outcome measure is improved compared to the baseline measurement or the control group but the difference is not statistically significant, the practice is classified as effective and not evidence-based.
