Local Health Data from Community Partners
Healthy Mendocino strives to provide the most updated health indicators for Mendocino County in a large variety of areas. However, given the time it takes to research and produce data for publication, oftentimes the most recent data is 1-2 years old. We have therefore dedicated a webpage to community partner program data to provide the most timely community health metrics. Let us know if you would like to contribute your program data to this central location. This will help increase collaborations, leverage funding with the most recent data, and keep us all informed.

Non-profit health systems, community-based organizations, and public health agencies across the country all share a similar calling: to provide public service to help improve the lives of their community. To live out this calling and responsibility, Adventist Health Howard Memorial, Mendocino Coast and Ukiah Valley conducts a Community Health Needs Assessment (CHNA) every three years, with our most recent report completed in 2022. Now that our communities’ voices, stories, and priority areas are reflected in the CHNA, our next step is to complete a Community Health Improvement Plan (CHIP), or as we refer to it in this report, a Community Health Implementation Strategy (CHIS).
