Impact: The Community Preventive Services Task Force (CPSTF) recommends self-measured blood pressure monitoring interventions combined with additional support to improve blood pressure outcomes in patients with high blood pressure. Additional support may include patient counseling, education, or web-based support. Economic evidence indicates that self-measured blood pressure monitoring interventions are cost-effective when they are used with additional support or within team-based care.

Goal: The goal of this immunization case management intervention is to improve immunization rates among infants of low-income, urban, African American families.
Impact: Immunization case management increased the knowledge of immunization schedules, rate of well-child visits, and up-to-date immunization rate for children of participating families.

Goal: The goal of the Northern Manhattan Start Right Coalition is to increase the timely vaccination coverage rate for the 4:3:1:3:3 series for children ages 19 to 35 months.
Impact: The Start Right Coalition increased timely vaccination coverage from 63.0% in 2002 to 96.8% in 2007.

Goal: The goal of the project was to increase the number of students current with school-required immunizations through utilization of the state immunization registry and increasing the number of parental consent forms received for immunization in School-Located Vaccine Clinics.
Impact: Of 2,015 children not in compliance with school immunization policies, 1,094 (54%) were brought into compliance through state immunization registry records or immunization in School-Located Vaccine Clinics.
