Goal: The goal of this intervention is to promote patient referral among patients with a recent STI diagnosis.

Impact: Program participants were more likely to report sexual partner notification at 1 month and were more likely to report no unprotected sexual intercourse at 6 months.

Goal: The goal of Focus on Youth is to teach youth the skills and knowledge they need to protect themselves from HIV and other STDs.

Impact: The Focus on Youth intervention increased self-reported condom use and positively affected perceptions six months after the end of the program.

Goal: The goal of the pilot was to assess the impact on health care use of addressing patients’ civil legal problems – the social, financial, or environmental problems that require assistance from lawyers to remedy. The lawyer was embedded in the health care team and present during case management discussions to identify specific civil legal problems and to help the team better understand how to address them. Additionally, this partnership provided civil legal aid services to patients when needed in a community health care system.

Impact: This pilot shows a medical-legal partnership for the super-utilizers of healthcare can lead to efficiencies within the health care system, reduce costs, and improve health outcomes among the most vulnerable patients.

Goal: New York City LegalHealth is a fruitful partnership that operates in 11 clinics of the city's public hospitals. One example is by supporting individuals in tenant-landlord disputes.

Impact: With an average of $225 per case, LegalHealth was able to effectively demand fixes in asthma patients' living conditions. This impact directly resulted in a 90% drop in emergency room visits and hospital admissions for this asthma patient group.

Goal: The long-term goals of this program are to decrease the rate of recidivism in families receiving social services, lower the rate of multiparent teenage pregnancies, reduce the rate of juvenile delinquency and alcohol abuse, and stop the intergenerational cycle of child abuse by teaching positive parenting behaviors.

Goal: The goals of the program are to 1) assist mothers in obtaining treatment, maintaining recovery, and resolving the complex problems associated with their substance abuse, 2) guarantee that the children are in a safe environment and receiving appropriate health care, 3) effectively link families with community resources, and 4) demonstrate successful strategies for working with this population and thus reduce the numbers of future drug- and alcohol-affected children.

Impact: At a 36-month follow-up women in PCAP were more likely to have received alcohol/drug treatment than the control group. Cost savings were suggested by a reduction in length of out-of-home care and prevention of Fetal Alcohol Syndrome.
