A Population Health Approach to Clinical Social Work with Complex Patients in Primary Care (Portland, Maine)

Goal: Patients with the highest medical and social vulnerability require a population-specific social work intervention in primary care to achieve positive medical outcomes and to decrease inefficient use of services, especially inpatient admissions and ED visits.
Impact: The study exhibits promise in decreasing inpatient visits and cost. The evidence also supports population-specific social work interventions integrated in primary care.
Addressing the Social Determinants of Health through the Alameda County, California, Place Matters Policy Initiative (Alameda County, CA)

Goal: The goal of the Place Matters initiative is to improve the health of participating communities by “addressing social conditions that lead to poor health” through “identifying the complex root causes of health disparities and defining strategies to address them.”
Impact: AC Place Matters has identified 10 best practices that could help other programs address social determinants of health: find and foster strong leadership, dedicate staff resources to the work, engage staff from across the local health department, contribute to building grassroots power, address root causes, partner with community organizations and leaders, partner with government institutions across sectors, work reactively and proactively, build capacity, and use tools that ensure a focus on health equity.

Goal: The goal of the Baltimore Healthy Carryout project was to increase healthy food options at carryout facilities and restaurants in Baltimore's low-income neighborhoods.
Impact: The BHC project reached 36.8% more customers during the intervention period than at baseline when comparing intervention carryouts to comparison carryouts. Customers reported purchasing specific foods due to the presence of a photo on the menu board (65.3%) or menu labeling (42.6%).

Impact: Metro Public Health Department of Nashville/Davidson County has implemented department-wide strategies to address existing health inequities.

Goal: California Healthy Start's mission is to provide comprehensive services that support the wide variety of needs of children and further student learning.
Goal: In this particular instance, the goal of the Undoing Racism Workshops was to enhance the capacity of the Michigan Department of Community Health's Bureau of Family, Maternal & Child Health to reduce racial disparities in infant mortality.
Impact: The Undoing Racism workshop helped to increase competency in the following topics:
-Racial prejudice and racism
-Racial privilege and power
-Institutional, cultural, and internalized racism
-Instiutional norms and practices
-Racial health disparities
-Social determinants of racial health disparities

Goal: The mission of Healthy Kids Express Dental is to improve oral healthcare in children through education on the importance of regular brushing, flossing, and using fluoride as well as through restorative treatments when necessary.
Impact: During the 2014-2015 school year, children pre-Kindergarten through 7th grade saw their average score on a dental knowledge test rise from 64.7% on the pretest to 72.1% on the posttest.
