The overall goal is to reduce the prevalence of diabetes and improve the care of people with diabetes by improving provider education.
The results indicate that a half-day site visit with an experienced diabetologist can lead to sustained, improved glycemic and lipid control in previously-uncontrolled diabetic patients. The online iDose tool provides an easy way for healthcare providers to calculate insulin dosage.

The goal of the program is to provide elementary schools with a low-cost, non-invasive curriculum to educate elementary school children on how to read nutrition labels, differentiate between marketing versus reality, and select healthier food options.
Nutrition Detectives shows that a low-cost, non-invasive educational program based around downloadable videos, presentations, and materials can improve young students' and their parents' ability to make healthier food and nutrition choices.

The goal of the Options/Opciones Project is to reduce or eliminate risky sexual and drug use behaviors of HIV-infected patients.
The Options/Opciones Project shows that a clinician-delivered HIV prevention intervention targeting HIV-infected patients can result in reductions in unprotected sex and that interventions of this kind should be integrated into routine HIV clinical care.

The Parent Enrichment Program is for families who are at risk for having their children removed from the home or whose children have been removed from the home due to abuse or neglect. The goal of the program is to enhance existing parenting skills, connect participants to needed resources, and support their goals related to social and economic self-sufficiency. Specific program objectives are to improve skills related to positive parenting and financial stability, develop family protective factors that guard against abuse and neglect, and reduce safety threats.

The goals of the program are to 1) assist mothers in obtaining treatment, maintaining recovery, and resolving the complex problems associated with their substance abuse, 2) guarantee that the children are in a safe environment and receiving appropriate health care, 3) effectively link families with community resources, and 4) demonstrate successful strategies for working with this population and thus reduce the numbers of future drug- and alcohol-affected children.
At a 36-month follow-up women in PCAP were more likely to have received alcohol/drug treatment than the control group. Cost savings were suggested by a reduction in length of out-of-home care and prevention of Fetal Alcohol Syndrome.

The goal of Pathways to Housing, Inc. is to provide independent housing, treatment services for physical and mental health, and employment to mentally ill individuals who are chronically homeless.
Pathways to Housing has received numerous awards for its projects in New York City, Philadelphia, and D.C. In addition to achieving large effects in terms of housing outcomes, compared with standard care in the community, Housing First also improves community functioning and quality of life.

The goal of the Phoenix Healthy Homes project was to use a multi-factorial approach to reduce hazard prevalence and improve self-reports of home safety and respiratory health.
The Phoenix Healthy Homes project showed that a tailored healthy homes improvement package significantly improves self-reported respiratory health and safety, reduces respiratory health and injury hazards, and can be implemented in concert with a mobile clinical setting.
